For all procedures, please be sure to arrive 30 minutes prior to your appointment!
Confirmation: We will call you to confirm your appointment at least 5 days in advance. Because of the current demand for this examination, there will be a $100.00 charge for any cancellation for a procedure within three full business days of your appointment date without a medical note from your family physician. Consultation appointments may be cancelled within one full business day of your appointment date. Cancellations of consultations within less than the stated time will be subject to a $50 fee without a medical note from your family physician.
Health Card: Patients must present a valid provincial Health Card or OHIP (Ontario Health Insurance Plan) Card at the time of their appointment.
Patient Information Form: Prior to your procedure, you will be asked to fill out a Patient Information form which will help us gather important contact information as well as medical history. You can complete the form on arrival or print the form and complete it before visiting our clinic.
Informed Consent: Prior to your procedure, you will be asked to sign a consent form. Please do not hesitate to ask us any questions at that time.
Sedation: If you choose to have sedation, please note it is strongly recommended that you do NOT drive or operate machinery, sign any legal documents, drink alcohol or return to work until the next day. We require that a responsible adult accompany you to the clinic and escort you home. We will not perform a procedure that requires sedation if you do not have a responsible adult to accompany you home.
